Our dental team of thirty staff members includes general dentists, hygienists, dental assistants, receptionists, and billing staff. We work together to provide comprehensive care in a 15 operatory clinic.

- Medical history information
- Insurance Cards (Medical Assistance or third party)
- Make sure you take all necessary medications, as instructed by your doctor.
In some cases, a patient’s care may be coordinated with an outside dental specialist. Specialties we commonly refer care for in these circumstances include oral surgery, endodontics, orthodontics, pediatrics, and periodontal care.
Orthodontic Program
An orthodontic program is available for Fond du Lac Enrollees (self-enrolled) under the age of 18. Our dentists determine which of their eligible patients are in need of orthodontic services. The orthodontist, dental coordinator, and one staff dentist then screen these patients at our annual Orthodontic Screening Day. Orthodontic need, as determined by the contracted orthodontist, is the main selection factor for the program. If selected, the patient would then have the orthodontic treatment completed at the orthodontist office.
