Home Health Services - Services

Foot Care:
This is a monthly clinic to help people who have difficulty reaching and caring for their feet. This clinic is most appropriate for people with diabetes, arthritis, obesity, and other health related concerns.

Health Care Assistants:
Health Care Assistants provide personal care such as hair washing, showers and nail care. They also will check vital signs, light housekeeping, meal preparation, and other services for clients with medical or health concerns, as assessed by a Registered Nurse.

Home Safety Assessment:
A Home Health Services staff member is available to perform a home safety check for elders or families who have a family member with disabilities. A limited number of safety items are available free of charge following the home safety check. Instruction on how to properly use equipment will be provided. Installation of safety equipment is not provided.

In-Home Care:
Specialized nursing care services are provided in the home including but not limited to medication management, wound care, dressing changes, and post-operative care. A Registered Nurse is able to provide medical care to the client, as ordered by the client’s physician.

Elder Picnic:
An annual picnic is held during the month of July on Big Lake in Sawyer. BINGO and prizes promote fun and a good time for all who attend.

Medication Management and Set-up:
This service is available to Elders who are on the Home Health Services Department caseload by the referral process and are taking multiple medications. A Registered Nurse (RN) and Licensed Practical Nurse (LPN) can help you categorize your medications, clarify dosages and assistant in setting them up for a 1-2 week time period.

Respite and Caregivers:
Services are for families with a homebound or disabled elder. A Health Care Assistant is able to provide personal cares and socialization while the care-giver runs errands, attends appointments, and other business during the day.
