COR-12 - Comprehensive Opiod Response with 12 Steps

Fond du Lac Human Services Division recognizes that many barriers exist for individuals addicted to opiates when engaging in treatment services and locating the appropriate resources. When medically appropriate, recovery may be assisted with certain medications which will allow the individual to engage in treatment services, access proper resources and learn necessary life skills.

Our interdisciplinary team, works closely with the client, and their family to recommend the appropriate pathway. These recommendations are based upon factors, such as:

  • Medical History
  • History of opioid use
  • Treatment and recovery history
  • Co-occurring Mental Health issues
  • Recovery environment and Support Network
  • Risk of relapse

Program Description

Purpose

The purpose of the Fond du Lac COR-12 program is to assist opioid addicted individuals overcome the initial barriers of recovery with the use of Buprenorphine/Naloxone (Suboxone®). Buprenorphine (Subutex®), and Naltrexone IM (Vivitrol®). FOL medical staff will stabilize a client with selected COR-12 medications. While the patient is being medically stabilized, they will be required to receive chemical dependency and mental health treatment from a Fond du Lac provider.

FOL COR-12 is not a maintenance program; all clients will be weaned off all COR-12 medications within a prescribed time limit that may be different for specific medications. The FOL COR-12 Program has been established to serve Fond du Lac medical patients exclusively.

The FOL COR-12 Program is a close collaboration between the Medical and Substance Use Disorder Departments and is conducted with the consent and cooperation of the providers within those departments.

Program Mission & Principles

The mission of the Fond du Lac Human Services Division is to elevate the health and social well-being of Indian people living in the service area through the provisions of services, research, education, and employment opportunities.

Eligibility

American Indian adult male or female living in the Fond du Lac service area. Must meet Fond du Lac medical requirements to participate in the COR-12 Program.

Confidentiality

Fond du Lac Human Services Division will treat the information pertaining to clients with the highest regard for confidentiality. Any suspected violations of patient privacy should be reported immediately.

Safety

The safety of the community, clients and staff are a primary concern. A client may have conditions placed on them by the FOL Police Department. Level Ill sex offenders are not permitted in the program. Clients will be supervised at all times while at the treatment center and during activities in the community. Threatening or violent behaviors are not permitted. Gang signs, sy mbols or gang indicators will not be tolerated by staff.

Recovery is attainable. The treatment program uses the person's strengths and culture - utilizing a Cognitive Behavioral and Motivational treatment model. This model moves the client into a better life away from alcohol and drugs and towards sober living. Engaging family, friends, and community is an important part of the healing process.

Participants will receive consultation and monitoring from counselors, case workers, physicians, physical fitness instructors, spiritual healers and/or clergy, probation officers and diet itians. Clients will undergo chemical dependency screening, mental health screening, medical/dental screening, nutritional screening, and criminal background checks.

Drug Testing

Clients participating in COR-12 will be drug tested at the start of the program to develop a baseline and daily after that.

3 Pathways

COR-12 consists of 3 pathways.

Pathway #1

No Medication Pathway. On this pathway, your withdrawal will be medically treated, when and if it is clinically appropriate with medications.

Pathway #2

Buprenorphine/Naloxone Pathway - Buprenorphine/Naloxone (Suboxone®) is a medication used to treat withdrawal and cravings while assisting your recovery and allowing you to engage in treatment and other supportive services over a longer period of time.

Pathway #3

Vivitrol/Naltrexone Pathway - Vivitrol (Nal trexone) is an opioid receptor blocker and is administered as a once a month intramuscular injection. This medication blocks the effects of opioids. It is not addictive. It does not replace or act as a substitute for opioids and cannot create a "high" like opioids.
